The discharge summary and screening is carried forward to iCHR, which is an engagement platform to record and monitor the immunization schedule, health, growth, and developmental profile of the child from birth to the age of 20 years.


The development of a child is a continuous process. Importantly , the first five years are very significant for a child’s brain development and their lifelong well being. Impaired health in those years can result in long-term implications for child as well as parents.
Thus, systematic child health surveillance is critical to reduce their morbidity and mortality rates. In India, child health and vaccination records are still maintained as paper-based records.
Also, error in manually plotting the parameters in growth charts results in missed opportunities for early detection of growth disorders in children. Complete immunization can ensure reduction in deaths. Missed dose or vaccination results in prolonged susceptibility towards these diseases which could cause untimely deaths.
Discharge summaries are essential documents to provide a long-lasting record of a patient's visit to a hospital. It provides an effective method of communication between various hospital services and primary care providers. Inaccessibility of these can result in unfavorable outcomes that may have a pronounced bad impact on the continuity of care, resources used, patient and physician satisfaction and above all patient safety.


Thus to address these persisting issues and fulfill the unmet need, we for the first time, to our knowledge, developed India’s 1st Hospital linked Integrated Child Health Record Cloud (iCHRcloud), a revolutionary concept of completely automating child’s vaccination record and crucial growth parameters to monitor their real-time development profile from birth to the age of 18 years.
iCHRcloud also has dual interface advantage as it provides a web based interface for the doctor & mobile application for the parents and thus, offer better transparent accountability to both the care providers and the consumers.
Milestone tracking as per WHO standard and identification of critical babies as per designed thresholds based on growth parameters would make iCHRcloud a more sustainable solution to significantly reduce the child mortality rate.
It’s smart prescription capability automates all the steps of prescription writing starting from chief complaints, clinical examination, investigation and final diagnosis in simple, user friendly and time efficient manner.


The architecture of the iCHRCloud system mainly consist of three parts i.e. the mobile interface, doctor module and cloud component.
The mobile interface caters to parents enabling child health record access on their smartphones. It allows parent to first register their child with a hospital or clinic and then accesses their child record via secure encryption over Internet. Patient registration requires unique patient identifier and mobile number with which patient is registered with medical facility. Post-authentication, parent can access the mobile interface, which is divided into sections related to child information; visit details, vaccination schedule, growth charts, prescription and settings.
The doctor module is available through browser to medical staff (doctor and nurses), which is setup on their OPD (Out Patient Department) desktop or laptop machine. Each medical staff is given credentials via which they log into system and access patients assigned per their OPD schedule. Each medical staff credentials are linked with their digital signature and profile. System maintains the availability of each doctor and its corresponding calendar based on patient appointments. During visit of a patient, its phenotypic measures like weight, medical staff records height and head circumference are recorded in application. Doctor uses the prescription section of iCHRcloud to capture complaints and prescribe medication. Each hospital or clinic has its individual data compartment and associated cloud subscription.
This cloud component allows each instance to maintain its own branding (logo, users, vaccination schedule and prescription templates), its own set of patients and users. Each instance is connected to respective HIS (Hospital Information System) and from where it pulls patient data at periodic interval (example: every 3 minutes). This enables patient data to appear on doctor module after patient makes payment.
